Registration and Immunization

If you need to register your son or daughter for school, simply stop by our school office to fill out the necessary forms. Please be sure to bring:

  • Registration form
  • Your child’s original birth certificate
  • A copy of your child’s immunization records
  • Proof of residency, such as a utility bill
  • Transcripts or files from a previous school, if applicable

Immunization Requirements

As a public school district, we follow the state guidelines for immunization requirements. Your child must be current on his or her shots in order to attend school. It is possible to get an exemption from the state if your personal or religious beliefs prohibit your child from receiving immunizations. We’ve included the list of immunizations your child needs as well as links to the exemption form and the Arizona Department of Health Services for you at the side of this page.
